A case of Focal nodular hyperplasia (FNH) vs Fibrolamellar variant of HCC at Olive Hospital Nanalnagar Hyderabad


Hyderabad, September 30, 2024: 26-year-old male patient presented with complaints of early satiety (no hunger) since 2 months, back pain on and off which increased since 1 month and was admitted at Olive Hospital Nanalnagar Hyderabad on 23 Sep under surgical gastroenterology.


There was no history of fever, nausea, vomiting, jaundice, Shortness of Breath, chest pain or other symptoms


Dr. Radhakrishna, Surgical Gastroenterologist on examining the patient found, a soft, vague mass in epigastric region with mild tenderness was diagnosed requiring further investigations

Ultrasound abdomen showed large heterogeneous mass lesion 12x10x11 cm with large exophytic (a raised lesion that grows outward from the surface of an organ or tissue) component attached to the liver..


CT abdomen was showed Focal Nodular Hyperplasia vs Fibrolamellar variant of Hepato-cellular Carcinoma, viz. a Tumour; Cholilithiasis.


Upper Gastro Endoscopy was performed to investigate if other organs and blood vessels are affected.


Blood tests were performed to investigate the nature of tumour and risk of bleeding.

After thorough evaluation, presence the Tumour in liver with gall bladder stone was diagnosed. A thorough pre-aesthetic evaluation was undertaken. The patient was then taken up for surgery LEFT LATERAL SEGMENTECTOMY + CHOLECYSTECTOMY on 24 sep. the surgery lasted over 4 hours, the tumour was successfully removed along with gall bladder stone with minimal bleeding. The patient had tolerated surgery well without any complications.


The patient is recovering well and is expected to be discharged within the next 2-3 days.

Timely diagnosis and preparedness of the anaesthesia and surgical teams facilitated the process.
